For workers compensation, please bring in your claim number along with your workers compensation insurance information. For personal injury claims, bring claim number, car insurance information or the insurance of the vehicle your were in. We do not accept third party. If you do not have personal injury insurance, we do accept your health insurance. If you do not have car insurance, health insurance, then you will need to retain an attorney.
